Civil-status extract (odpis) translation
Sworn translation of odpis aktu urodzenia, małżeństwa or zgonu — when the office asks for an extract, not a booklet certificate, often with a recently issued date.
Odpis aktu stanu cywilnego is an official register extract. Polish USC offices often ask for it specifically (sometimes “not older than N months”), not a laminated booklet. Translation is sworn.
Odpis vs certificate: what to translate for a Polish USC
How does an odpis differ from a booklet certificate?
It is a current register extract as of the issue date.An odpis has different form details, series and extract issue date. For transcription in Poland, the odpis is what most often appears in USC instructions.
Full (zupełny) or short (skrócony)?
A full extract has more fields.A short multilingual odpis is sometimes accepted without translation (Convention No. 16). A full Ukrainian/Russian odpis needs a sworn translation. Do not order a translation “just in case” until you see the form.
Extract freshness
Some offices set a 3–6 month window.Translation does not extend the odpis validity. If the issue date is running out — get a new extract first, then translate the same day.
One template — different events
Birth, marriage, death — separate service pages. This page is about the odpis format. If your intent is specifically marriage or a child’s birth certificate, use the narrow URLs: different FAQ and filing scenarios.
What stays at USC
The foreign odpis + translation often are not returned. At transcription documents are filed with USC acts. Make copies for yourself before the visit.
Apostille on an odpis
A typical requirement before transcription. The stamp is affixed to the extract in the issuing country, then comes the sworn translation of the odpis and, if needed, the apostille page.
Languages for odpis aktu translation
We work both ways: a foreign document into Polish for offices in Poland, a Polish document into the destination-country language.
Documents from Ukraine for offices in Poland — the most common pair.
Soviet-era documents and those issued in Russia and Belarus.
Documents from the UK, Ireland, the USA, India and the UAE.
Polish documents for Ukrainian offices — certificates, extracts, rulings.
For foreign employers, universities and consulates.
For offices and banks in countries that require Russian.
Three types of translation — and when you need each
If the document goes to a public office — you need a sworn translation. Prices below are for Ukrainian, Russian and English; other languages are quoted separately.

Who already ordered
All reviewsI sent a photo in the evening, had a PDF by morning, and the paper copy in Wrocław two days later. Urząd Wojewódzki accepted it without questions.
They asked to match the name spelling to the passport — I would not have thought of it. That is why the pack was accepted first time.
The amount quoted on Telegram did not change. Good when there are no “extra pages” at the end.
FAQ
These are the questions we hear most about this document.
For some UW offices — no. For USC transcription they almost always ask for an odpis with a fresh date. Check the office list before ordering a translation.
