Academic transcript / wykaz ocen translation
Sworn translation of an academic transcript — when there is no diploma yet, studies are unfinished, or the university asks for a subject list separately from the diploma.
A wykaz ocen lists completed courses, credits/hours and grades. It is requested for transfers, unfinished studies or when the diploma has not been issued yet.

Why not a diploma?

For filing with a Polish university, apostille on the transcript is a common requirement — as with a diploma.
