Death certificate translation
Sworn translation of akt zgonu for inheritance at a notary, ZUS benefits, closing accounts and death transcription at a Polish USC.
A death certificate in Poland is needed where the fact and date of death must be proven officially — inheritance, pension payments, banks, USC transcription. Offices accept a sworn translation.
Sworn death certificate translation for inheritance and ZUS
Why does a notary need a death certificate?
To open an inheritance case.Without proof of the decedent’s death the notary will not start the procedure. The translation must include the full name, date and place of death as in the original.
ZUS and benefits after death
Pension, supplements, funeral benefits.ZUS asks for an official death document with a translation. This is not a child’s birth certificate or a marriage act — a different set of fields and checks.
Death transcription at a Polish USC
Entering a foreign act into the register. As with birth/marriage, USC takes a foreign odpis plus a sworn translation. Originals often stay in the file and are not returned.
Medical death note vs civil-registry act
Offices usually need the civil-status act.A medical pronouncement of death is a separate document. For inheritance and USC use the akt / odpis zgonu from the registering authority.
Urgency
Inheritance and bank deadlines are often tight. We can deliver faster after manager confirmation. PDF is sent immediately; paper by courier across Poland.
Apostille
Often for notary and court. For USC transcription and some inheritance acts an apostille on a foreign death certificate is requested more often than for internal employer certificates.

Who already ordered
All reviewsI sent a photo in the evening, had a PDF by morning, and the paper copy in Wrocław two days later. Urząd Wojewódzki accepted it without questions.
They asked to match the name spelling to the passport — I would not have thought of it. That is why the pack was accepted first time.
The amount quoted on Telegram did not change. Good when there are no “extra pages” at the end.
FAQ
These are the questions we hear most about this document.
Yes if all fields and seals are visible. For a notary an official odpis is better — we will say if the photo quality is not enough.
